Palliative care is specialized care for people with serious illnesses.

It focuses on providing patients with relief from the symptoms and stress of a serious illness. The goal is to improve the quality of life for both the patient and the family.

No doubt living with a long-term condition can be extremely challenging for the individual themselves, as well as their families. Under such circumstances people may need support at many levels. Caring for a loved one who is on the end of life pathway can also be very painful and stressful for the family and friends.

Our palliative care service aims to support you during this difficult time. Our professional carers are trained to provide support to clients requiring palliative care, and make life more comfortable for everyone involved.

At True Homecare we provide palliative care by working hand in hand with a team of doctors, nurses and other specialists. We work together with our clients’ doctors to provide an extra layer of support, and also liaise with the clients’ family and friends.

We provide a person centered service by delivering care in a person-centred way, taking account of the circumstances, wishes and priorities of the client, their family and friends. We devise a care plan taking into account our clients’ needs, choices and preferences. We provide consistence of care, by assigning a small team of carers who work with the client on a regular basis.

Our carers are fully trained in end of life care and are supported to develop further knowledge and skills.
We firmly believe that everyone should have the right to die at home – if that is their choice. In order to make this possible, we work with other health and social care professionals to help provide a comfortable end of life, palliative care to our clients.
