A senior carer visits to understand the client's condition, wishes, current clinical support, and the family's own capacity, usually within 48 hours of first contact.

Care Plan Development

We build a written care plan in consultation with the family and, where possible, the district nursing or hospice team, covering routines, preferences, and emergency contacts.

Carer Matching

We select a small, consistent team based on the client's personality, interests, and specific care needs, rather than assigning whoever is next on a rota.

Ongoing Review

Care plans are reviewed regularly and updated immediately after any significant change in condition, in direct contact with the clinical team involved.

24-Hour Support

Families have a direct line to our care coordination team at any hour, so an overnight change doesn't have to wait until the next scheduled visit to be addressed.

Supporting Advance Care Planning
